The Core Institute is the largest national Alcohol and Other Drug (AOD) database about college student's drinking and drug use in the country. We offer a comprehensive range of cost-effective surveys that measure risky behaviors. Below are samples of our surveys. Click on the survey title to download a sample survey.

Short Form
- Assesses the nature, scope, and consequences of alcohol and other drug use on college campuses.
- (23 questions/15-20 minutes to complete)

Long Form
- It contains all the features of the Short Form but also assesses the students' attitudes, perceptions, and opinions about alcohol and drugs.
- (39 questions/20-30 minutes to complete)

Community College Form
- It contains all the features of the Long Form but has been redesigned specifically for community college campuses.

- Assesses students' use of alcohol, marijuana, and other illicit drugs along with extensive perceptions of alcohol and drug use by friends, peers, and other student groups on campus.
